Lab Survey Fact Sheet

 

1.  A survey must be conducted of all storage areas.

 

2.  Surveys are required based on frequency of usage:

 

Monthly:  < 200 uCi per experiment or < 1 mCi per week

OR

Weekly:  > or = 200 uCi per experiment or > or = 1 mCi per week

 

 

3.  If you choose the option “Close-out survey” when you document your lab survey; that will also result in a permit amendment to remove the room from your permit.  Use caution when selecting this option.  Additionally, close-out surveys are more substantial than routine surveys.  More swipes are required to ensure the room is free of contamination and may be released for unrestricted use.

 

4. Associate users with log-in access may document lab surveys.

 

5.  If a deficiency is noted during the quarterly review by one of our Health Physicists, it will be flagged in the “My To Do” portion of your menu.  You should address the deficiency (and resubmit, if necessary), prior to submitting a new survey.

 

6. The web-based database will serve as your new warehouse for your surveys.  All surveys that are submitted to the system will not be reviewed.  In an ideal world if you submit 12 surveys for the year, 4 of them will be reviewed by RSS for quality assurance and compliance.
